Abstract:

Introduction: Shoulder pain is a very common condition in society, characterised by loss of mobility, pain and functional impairment. The acromioclavicular joint is one of the four joints involved in shoulder movement, yet in most cases it is overlooked when planning a treatment protocol. Methodology: Randomised, controlled, double-blind clinical trial. Acromioclavicular manipulation was performed using the SNAP technique, assessing its influence on parameters such as range of motion (ROM) in abduction, pain, and data from ultrasound examination, and comparing this with a placebo. Descriptive and inferential analyses were carried out using the appropriate hypothesis testing methods, with a p-value of <0.05 considered statistically significant. Results: The samples were homogeneous for the independent variables of age (p=0.521) and height (p=0.079), but not for weight (p=0.019). Significant differences were found between the pre- and post-treatment measurements in the two groups for the variables ROM (p<0.01), pain (p<0.01) and the ABD test (measured using a visual analogue scale) (p<0.01). In contrast, the ultrasound variables were not significant (p>0.05). Conclusions: The SNAP technique is effective in increasing mobility and reducing pain in the treatment of patients with painful shoulders. Further studies are needed to explain these findings through ultrasound analysis. (translated by Ostlib)

Introducción: El dolor de hombro es una patología muy frecuente en la sociedad, caracterizada por pérdida de movilidad, dolor e incapacidad funcional. La articulación acromioclavicular es una de las cuatro que participan en el movimiento del hombro, siendo en la mayoría de los casos olvidada en la planificación del protocolo de tratamiento. Metodología: Ensayo clínico aleatorizado, controlado, doble ciego. Se ha realizado la manipulación de la acromioclavicular con la técnica de SNAP valorando cómo influye en parámetros como ROM en abducción, dolor, y datos procedentes de la exploración ecográfica, comparándolo con placebo. Se ha realizado análisis descriptivo e inferencial con los test de contraste de hipótesis correspondientes, asumiendo un valor de p<0,05 como estadísticamente significativo. Resultados: Las muestras son homogéneas para las variables independientes edad (p=0,521) y altura (p=0,079), no así para el peso (p=0,019). Se encuentran diferencias significativas entre las mediciones pre y post entre los dos grupos en las variables ROM (p<0,01), dolor (p<0,01) y el test en ABD(medido con escala visual analógica) (p<0,01). En cambio, las variables ecográficas no son significativas (p>0,05). Conclusiones: La técnica de SNAP es efectiva para aumentar la movilidad y reducir el dolor en el tratamiento de los pacientes con hombro doloroso. Hacen falta más estudios para poder explicar estos hallazgos mediante análisis ecográfico.
