Abstract:

Osteopathic treatment of enuresis nocturna must be based on knowledge of the specific anatomical and physiological dynamics of bladder function. Diagnostic competence is closely linked to comprehension of pathophysiologic processes. Osteopathic assessment, however, includes still different approaches. Taking into account osteopathic literature, the following mechanisms of dysfunction as well as facets of potential osteopathic treatment will be discussed: The mechanism of micturition; nerval innervation; limitation of direct or indirect gliding mobility of e.g. peritoneal relations; impaired mobility of local ligamentous structures; insufficiency of pelvic floor muscles; interrelation pattern of diaphragms; somatic dysfunction of the spine, the pelvis, and the lower extremity; respiratory obstruction; obstipation; congestive phenomena of the pelvis; effects of arterial and endocrine processes. Currently, osteopathic treatment fails to provide evidence in treating Enuresis nocturna.

Die osteopathische Behandlung der Enuresis nocturna setzt Kenntnisse der spezifischen anatomischen-physiologischen Dynamiken der Blasenfunktion voraus. Diagnostische Kompetenz ist eng verbunden mit dem Verständnis pathophysiologischer Prozesse. Die spezifisch osteopathische Diagnostik ist davon jedoch zu unterscheiden und basiert auf anderen Zugängen und Befundungsmethoden. Folgende Dysfunktionsmechanismen und Aspekte einer möglichen osteopathischen Behandlung werden unter Berücksichtigung osteopathischer Literatur diskutiert: Miktionsmechanismus, nervale Innervation, Einschränkung direkter und indirekter Gleitbeweglichkeit z.B. peritonealer Relationen, Mobilitätseinschränkung lokaler ligamentärer Strukturen, Beckenbodenschwäche, Beziehungsmuster der Diaphragmata, somatische Dysfunktion der Wirbelsäule, des Beckens und der unteren Extremität, Atemwegsobstruktionen, Obstipation, Stauungserscheinungen des Beckens, arterielle und endokrine Einflüsse. Ein Erfolg osteopathischer Behandlung bei Enuresis nocturna kann gegenwärtig nicht belegt werden.
