Abstract:
Objectives To review a series of recent articles on the distinct manual techniques used in manual osteopathic treatment and their impact on muscle proprioceptors. Material and method We performed two searches: a broad search of the literature in the English language, using the MEDLINE database (1990-2007), and a manual search of books on manual therapy and osteopathy. Results High-velocity low-amplitude (HAVLA) spinal manipulation generates excitement in the intrafusal fibers, mainly primary endings, resulting in a decrease of the H-reflex and the Ia-MN synapse, thus decreasing electromyograph activity in symptomatic patients. In addition, the central motor system is facilitated and blood cytokine levels decrease. Muscle energy techniques and myofascial stretching have a greater effect in activating the inverse myotatic reflex on Golgi tendon organs and in generating a decrease in the H-reflex. Conclusions The manual techniques used in osteopathy have a limited but important impact on muscle proprioceptors. In general, there is a significant trend toward balancing the activity of the alphamotoneuron due to distinct neurological mechanisms that can cause somatic dysfunction.
