Abstract:
Aim: to identify additional benefits of osteopathy (O) in standard bladder-training (BT) for women with overactive bladder. Study design: pragmatic clinical randomized controlled trial Methods: intervention group (IG, n=17) and control group (KG, n=16) received five BT sessions. IG received four to nine additional sessions O. Target parameters: questionnaires about symptoms (ICIQ-OAB) and symptom-related quality of life (ICIQ-OABqol). Measurements: M1 before, M2 after four-month treatment phase, M3 (follow-up) after another two months. Results: In both groups significant improvement of symptoms (IG p=0.003, KG p=0.001) and quality of life (IG, KG p=0.001) could be achieved from M1 to M2. The treatment success persisted until follow-up. No significant difference of symptoms (ICIQ-OAB: M1 p=0.123; M2 p=0.201; M3 p=0.118) and symptom-related quality of life (ICIQ-OABqol: M1 p=0.652; M2 p=0.290; M3 p=0.278) could be observed between IG and KG in any of the three measurements. Significance level 5%. Discussion: a general additional benefit of osteopathy could not be demonstrated. Only after post-hoc division of the patients in three cause groups according to osteopathic findings, tendencies for each adequate, cause-specific treatment can be observed: local dysfunctions (n=5): osteopathy; psychogenic background (n=18): osteopathy, plus psychodynamic, trauma-therapeutic elements; misconceptions/wrong behavioral patterns (n=6): bladder-training. Conclusion: follow-up studies with larger samples and randomization in three cause groups are necessary to confirm an additional benefit of osteopathy. An overactive bladder requires cause-specific treatment.
