Abstract:
Background: Extubation failure due to larynx lesions is one of the adverse effects of children who need mechanical ventilation (MV), which increases morbidity, mortality and pediatric intensive care unit (PICU) length of stay. Preventive methods for laryngeal edema, such as Continuous Positive Airway Pressure (CPAP) and corticosteroids, have shown efficacy, but they have adverse effects. Osteopathic manipulative treatment (OMT) is a manual therapy that has been shown to reduce hospitalization time in premature infants. Objectives: To propose a new OMT protocol and to evaluate the evolution of the extubation failure rate, length of hospital stay, and time on mechanical ventilation in infants who participated in the protocol. Methods: controlled, randomized, double-blind study. Participants were divided in intervention (G1=OMT) and control (G2=placebo) groups. Interventions were performed twice a week until PICU discharge. Results: The sample consisted of 80 infants, G1=41 and G2=39. The OMT group showed a significant decrease in extubation failure rates (G1=3; G2=17; p=0.001). Mechanical ventilation days (G1= 9[6-13]; G2= 9[6-12]; p=0.996) and hospital length of stay (G1= 14[10-20]; G2=13[10-18]; p=0.242), did not show difference between groups. Conclusion: The results of this study suggest that OMT can contribute to reducing extubation failure in infants admitted to the PICU requiring mechanical ventilation.
