Abstract:

Objective: We tested the conditions under which the SFT is performed to evaluate its reproducibility and diagnostic value, points on which osteopaths do not agree. Methods: We performed SFTs on 256 asymptomatic volunteers, standardizing the subjects’ position with defined foot spacing across 16 successive tests, conducted by two blinded testers. Results: The agreement rates of 57.8% and the mean Kappa coefficient of 0.12 are influenced by the subject’s foot position and gender, and are only minimally influenced by the tension of the hamstrings. The reliability of the SFT is twice as high when performed in a standardized position in male subjects, where the highest kappa coefficient was found, κ = 0.37 [0.18–0.57] (p < 0.001). The kappa coefficient was not significant when the SFT was performed on female subjects. Conclusion: Due to its simplicity and the fact that it yields a positive result in nearly 60% of cases, the SFT is a relatively reliable step in osteopathic clinical diagnosis, with low inter-examiner agreement provided that the subject’s foot spacing is standardized to approximately 20 cm, rather than allowing them to assume a spontaneous position, which reduces its reliability. The SFT can remain one of the first tests performed during the patient’s clinical examination but must be supplemented by other tests. (translated by Ostlib)
